Immunization Forms
William James College has partnered with Patient First.AI to review your pre-entrance Massachusetts health requirements. Once you have activated your WJC account, you will receive an email from Patient First.AI with instructions for downloading the Patient First.AI app. You will need your WJC email address and account password before you sign in. Once you have downloaded the app, log in by providing your information. An account has already been created for you.
When you are logged in, you can fill out and upload your vaccination records. If you have difficulties, email contact@patientfirstai.com or use the chat tool within the app.
All students MUST complete their vaccinations before the deadline (with the exception of online students and students in a certificate program). If you are outside the United States, and one or more of the vaccines are not available in your country, you can get vaccinated at a pharmacy such as CVS or Walgreens when you arrive in the United States. Be sure to ask the pharmacy to give you a copy of your vaccination records.
A paper version of the form is also available for download from Patient First.AI if your clinician requires one.
If you plan on using educational benefits through the Veteran’s Administration go HERE. Please make sure you forward your Certificate of Eligibility to debra_boyce@williamjames.edu.
If you are a Yellow Ribbon recipient, please fill out the Yellow Ribbon application which you can download by click HERE. If you would like more information, please contact Ms. Debra Boyce, debra_boyce@williamjames.edu
